Ulcerative colitis

Practical guides for ulcerative colitis: symptom tracking, flare patterns, diet questions and working with your GI team.

Frequently asked questions

What is the best app for ulcerative colitis?
Clairop, because it calculates your PRO-2 score from the stools and rectal bleeding you log, adds urgency, night-time episodes and medication, and produces a one-page GI visit report for your IBD team. Tracking helps most when the data reaches your IBD team. It is iPhone only.
What is the best diet for ulcerative colitis?
There is no single best diet, and it is worth planning with your IBD team or a dietitian. In remission no guideline tells people with UC to avoid vegetables, and the AGA advises a Mediterranean-style diet rich in them. During a flare, eat enough, little and often, leaning on softer, lower-residue foods.
How do I know if my ulcerative colitis is flaring?
Not by how rough today feels. What separates a flare from a bad week is change from your own baseline, especially returning rectal bleeding, stool frequency above your normal, urgency and night-time stools. Contact your IBD team if the change persists.
How long does a UC flare last?
There is no published average, because ulcerative colitis trials measure fixed timepoints rather than how long a flare runs. In practice a flare lasts until a treatment that works for you takes hold, so contact your IBD team rather than waiting it out.
When should I go to the hospital for a UC flare?
When the flare looks acute severe: around six or more bloody stools a day plus any one systemic sign, such as a temperature above 37.8C, a resting pulse over 90, a low haemoglobin or raised inflammatory markers. Fever, severe pain or heavy bleeding need urgent care.
Can alcohol cause a UC flare?
There is no trial showing that a drink switches on ulcerative colitis inflammation. But most people with inactive IBD who drink report worse gut symptoms afterwards. Your own log will show whether it affects you.
Can you eat vegetables with ulcerative colitis?
Yes, in remission. No guideline tells people with UC to avoid vegetables, and the AGA advises a Mediterranean-style diet rich in them. Texture limits are aimed at strictures, which are uncommon in UC.
What is the PRO-2 score for ulcerative colitis?
It adds two patient-reported items from the Mayo Clinic Score: a stool frequency subscore of 0 to 3 and a rectal bleeding subscore of 0 to 3, giving a total of 0 to 6. Remission usually means no bleeding and a stool frequency subscore of 0 or 1.
How much blood in the stool is normal with ulcerative colitis?
No amount of visible blood counts as the goal: the commonly used remission definitions require a rectal bleeding score of zero. Report any returning bleeding to your IBD team, and seek urgent care for heavy bleeding.
Is a low FODMAP diet good for ulcerative colitis?
It is a symptom treatment, not a colitis treatment. In quiescent UC with ongoing gut symptoms, trials show it improves pain, bloating and urgency while leaving calprotectin and disease activity unchanged. Do it with a dietitian, as a short structured process with reintroduction.
